God damn. I want to punch them in the mouth, then in the throat. Then break their ankles so they can't follow me as I walk away to somewhere with people who don't use Internet jokes in non-Internet conversations.
Related: I hate people who make references to things constantly. Or when you reference something, or mention something in conversation, and then they are like OH MY GOD OH MY GOD and act like it's some huge fucking deal that they know what you're talking about because you're both in the super secret Internet 4chan club.
For example: one time a few years ago I was in a computer lab and the teacher mentioned Futurama. Suddenly like two or three douchebags all go "Oh my god Futurama I watch Futurama too!" like it's some big fucking deal that they aren't the only ones who watch a television show that's on during prime-time and is watched by millions of people.

I thought I myself was a hardcore Net addict; I know my way around, I've seen memes live and die, etc. But I always I try to make sure whatever's on the net doesn't tag along with me in real life. And I thought other people would do the same.

I occasionally use 4chan memes in spoken conversation. Several of them are amusing without prior knowledge, because the context is obvious (man the harpoons, etc.) Around my one /b/tard friend I will make more oblique references (it's awwwwright.) Usually we're making obscure philosophy/historical/scientific referential jokes anyway, so it's not like it makes the conversation much harder to follow.

I know a few /b/tards in rel life, and we hardly ever use memes, in any context. The only times I can remember are once when we were all playing Pokemon (lol) and one said to me "I heard you like Mudkips", and once when we were all drunk and walking past a construction site, I climbed up on a steamroller and shouted "WRYYYYYYYYYYYYYYYYYYYY". Oh, and we all sing along to the Fresh Prince of Bel-Air theme song whenever it's on, but that's less of a 4chan thing and more of a catchy song thing.
What's funny is, me and the one other person that visists 4-ch I know use 2ch and 4ch terms a lot. Referring to Koreans as nidas, singing DQN songs, calling things VIP QUALITY, etc.
None of the people I know in real life are chantards like me, but a few have been either introduced to memes by me or by sites with strong exposure to the *chans both of us attend, so we occasionally use 4chanisms when talking to each other. The occasional "lol" or "XD" (the latter pronounced as in the two letters) comes naturally though...

I've only recently moved away from 4chan's /b/. Having used it for over 3 years I finally grew tired of stupid memes repeated over and over again. Plus the massive influx of trolls and newfags have ruined it completely.
However, the kind of people using internet slang in real life are exactly the people /b/ originally hated (and mostly still does). The reasons can be compared to hating weeaboos (which is now often misinterpreted to liking anything Japanese).

There's a few that I use, but not to be clever or cool (in fact, it's pretty uncool, I think). I just think they're funny-sounding, whether you're 'in' on the joke or not.
The ones that I use are, "Ugly girl is ugly" "Creepy house is creepy" etc., "I has a ___" (after "I has a shuvel"), "how do I [something in past tense]?", and the word "animu" for "anime". I don't use them very much when I'm talking to people, but they definately turn up in my internal monologue alot.
Oh, and I've adapted the word weaboo into "gothaboo" to describe one of my friends, who is a wannabe goth and is heavily into anime and wapanese-ness. She's hot, though. Goth Loli clothes may be weird to wear in public, but...I'd do her.
