haha. fyi, the doctor's actually asking you to bear down (like when you take a crap) to check if you have a hernia poking through...it's just easier, not to mention more dignified, to ask a patient to cough. plus, you don't even realize what you're doing.
you could try bearing down and going ERGGGHHH as loud as you can next time he asks you to cough...tell him Anonymous sent you
