The first time I ever saw it was on /vt/, in the form of a comic mocking anons there for dramamongering and theorizing about what interpersonal strife is happening behind the scenes based upon subtle, probably meaningless words and behaviors of various vtubers. There, it was basically presenting "/vt/ sisters" as behaving like gossiping teen girls while having adult male bodies. Ironically, /vt/ is probably one of the most female boards outside of /cgl/, so there's a decent chance that the people that that comic was mocking were, in fact, actual teen girls and 20something women. Outside of /vt/ and more recently on there it's probably just a mocking way to imply someone is trans or trans-adjacent.
